Preoperative chemotherapy in the treatment of esophageal carcinoma


Abstract
This study was performed to demonstrate some date concerning prospective clinical assessment on preoperative chemotherapy for treatment of esophageal carcinoma. Author had studied 60 cases with resectable intrathoracic esophageal carcinoma
treated
at
Department of Surgery, Queen Mary Hospital From Dec. 1989 to May 1991. Of all 60 cases, 29 cases was treated with operation after chemotherapy (chemo group) and 31 cases was treated with operation only (surg group).
@ES The results obtained were summerized as follows :
@EN 1. The mean age was 65 years old in chemo group and 62 years old in surg group. The ratio of male to female was 26 : 3 in chemo group and 26 : 5 in surg group.
The longest lengths of tumor were 7.2 cm in chemo group and 6.8 cm in surg group according to barium esophagogram, and 6.1 cm in chemo group and 5.8 cm in surg group according to endoscopy. There were no statistical differences between both above
clinical characteristics.
2. The response to chemotherapy was complete response in 4 cases, partial response in 7 cases, minimal response in 12 cases, progression of tumor in 2 cases, and not evaluated in 4 cases out of all chemo group.
The complications of chemotherapy were leukopenia in 8 cases, thrombocytopenia in 5 cases, pancytopenia in 1 cases, renal impairment in 7 cases and others in 3 cases of all 29 cases. But there was no severe morbidity and mortality after
chemotherapy.
3. The resectable cases were 23 cases in chemo group and 28 cases in surg group and there were no statistical differences in operative stages, operative morbidity and operative mortality between both group (p>0.05).
The overall cumulative survival rates and median survival times were similar in both group(P>0.05). but they were higher and longer in chemo group (median survival?>22.0 months) than surg group (median survival = 10.8 months).
4. According to above results, it seems to be that preoperative chemotherapy in esophageal carcinoma is effective treatment modality because of minimal side effect and high response of chemotherapy and better survival rate in curative resectable
cases
than surgery only cases.
